Photos are rejected when they break authenticity, explicit-content, privacy or safety rules.

Moderation is not perfectly simultaneous. Another violating image being visible does not mean your rejected image was acceptable.

Warnings usually mean content or behavior may break policy. Review the message and change the problem before further action is taken.

Keep one primary account and request closure of unused accounts through the account or support flow.
